Risk Management

The Risk Management Department of Long Beach Unified School District is dedicated to promoting a safe, healthy, and secure environment for all students, staff, and visitors. Our team works proactively to identify, assess, and mitigate risks that could impact the District’s operations, assets, and overall well-being.

Risk Management services include employee safety programs, workers’ compensation, liability claims management, insurance coverage, and health benefits. We also collaborate with departments across the District to support the health and safety of our school communities.

Property & Liability Claims

The Claims for Damages Form is used to formally present a claim against the District for vehicle damage, property damage or personal injury.  Submission of this form does not constitute an admission of liability by the District, nor does it guarantee acceptance, approval, or payment of any claim. Individuals are able to access the form by requesting that a copy be emailed to them by contacting the Risk Management Department at RiskManagement @lbschools.net or (562) 997-8234. All claims must be submitted in compliance with statutory requirements, including applicable filing deadlines, and will be reviewed and processed consistent with governing laws and District policies.

Requesting the form by email:

  • Individuals may contact the Risk Management Department to request that the Claims for Damages Form be emailed to them.
  • Requests should include the requester’s name and preferred email address to ensure timely processing.

Certificates of Insurance

 Certificate of Insurance (COI) may be requested when verification of the District’s insurance coverage is required for official District business. Requests must be submitted in advance to allow adequate processing time and must include complete information regarding the requesting party and the purpose of the certificate.

To request a Certificate of Insurance, please submit the following Form to the Risk Management Department.  All requests are subject to review for compliance with District policies, contractual obligations, and coverage limitations. Submission of a request does not guarantee issuance of a certificate or modification of coverage.
