There are several online forms found in the Employee Self-Service tile on the District Portal page.  To access this page, click on the following link: District Portal.

The following is a list of items you will find:

  • Direct Deposit Authorization
  • Salary Advance
  • W-4 Form
  • Employee Address, Email or Telephone Change

In-Person Forms

  • Address/ Name Change
    (Please note that Name Changes require the submission of a new social security card and a valid government-issued photo ID.  Please bring these documents along with the Name Change form to Human Resource Services).
