I understand and agree to comply with the following statements:  

• Only one cover sheet needs to be submitted each time your work is submitted. Each additional time work is submitted a new coversheet is required, but please use the same four digit code(your last four digits of your student id number).
• All literary works submitted become the property of the Visions Magazine and will not be returned. Of course, you still own the work itself. Artwork and photographs may be picked up from room 406 after Visions has been released

      (May '04). This is solely the artist's responsibility and must be done on or before 

      June 4^th or the work will be discarded.

• All writing and artwork must be original. Plagiarism of any kind will not be published and appropriate disciplinary measures will be taken.

I give permission for any and all works I have submitted to Visions Magazine to be published in the 2003-2004 edition.  Further, I understand that no royalties or discounts will be awarded to me, should my work be featured in the magazine.
